Do not Leave Revenue on Table | Collect your Deserved Cash

Occupational therapy billing requires specialized knowledge of OT-specific coding, Medicare thresholds, evaluation complexity, and documentation requirements. Our OT billing services help reduce denials, prevent underbilling, and support compliance with payer regulations. From eligibility verification to denial appeals, we align billing with the care your practice delivers.

You are Losing Money without Realizing it

Occupational therapy revenue rarely vanishes in a single dramatic denial. It erodes through small coding decisions, missed modifier applications, and documentation gaps that individual claims pass through unnoticed—until the pattern becomes a cash flow problem.

Timed Unit Miscalculations

Every time-based CPT code must reflect actual skilled treatment minutes, and the 8-Minute Rule converts those minutes into billable units with strict thresholds. Medical coding for two units when documentation supports one invites denials and audit exposure.

Modifier Errors and Omissions

The KX modifier attests that continued care beyond Medicare’s threshold is medically necessary. The GP modifier identifies therapy services. The CQ and CO modifiers designate assistant-delivered care at reduced reimbursement rates.

Plan of Care and Certification Gaps

Medicare and commercial payers require a signed, dated plan of care with physician certification within defined timelines. Claims submitted without a current POC or recertification face denial regardless of clinical appropriateness.

Documentation That Fails Medical Necessity Review

Payers do not pay for treatments. They pay for documented skilled needs. Notes that describe what was done without explaining why it required a licensed therapist and why progress justifies continued care create denial risk on every claim.

Denial Resolution & Patient Billing

Recover missed revenue and spend less time chasing claims. Let your team focus on helping patients move better.

Why Choose Smart RCM Billing as Your Physical Therapy Billing Company

The physical therapy billing company you select becomes an extension of your practice’s financial operation. Smart RCM Billing earns that position by doing three things better than the alternatives.

Therapy-Only Focus

We are not a multi-specialty biller that treats PT as one line item among dozens. Our team works in physical therapy billing every day.

Full Denial Ownership

Many physical therapy billing companies re-bill denied claims or send documentation errors back to clinic staff. We do not.

Transparent Performance Reporting

You receive monthly reporting on clean claim rate, days in A/R, denial trends by payer and reason, and collection performance against expected reimbursement.

What Our Physical Therapy Revenue Cycle Management Process Delivers

Our RCM process follows a defined sequence designed to prevent revenue loss rather than recover it after the fact.
1

Confirm Coverage

We verify eligibility, benefits, visit limitations, and authorization requirements before the first scheduled appointment.

2

Validate Authorization

We track authorization status across the full plan of care, monitoring approved visits, expiration dates, and recertification deadlines.

3

Code Against Therapy-Specific Rules

Every claim is coded by professionals who work exclusively in physical therapy billing.

4

Scrub Every Claim

Our claim scrubbing process checks for documentation sufficiency, modifier presence, unit accuracy, plan of care currency, and diagnosis-to-treatment alignment.

5

Submit Clean Claims

Clean claims go out promptly. We do not hold claims for batch convenience or administrative delays that extend your days in A/R.

6

Post Payments

We post payments accurately, compare reimbursements against contracted rates, and flag underpayments for follow-up rather than writing them off silently.

7

Resolve Every Denial

Denials are investigated, corrected, and appealed by our team. We track denial patterns by payer and reason code to eliminate recurring root causes.

8

Report Performance

Monthly reporting shows clean claim rate, A/R aging, denial trends, and collection performance. We use that data to refine workflows quarter over quarter.

Keep Your Revenue Cycle Moving

Changing billing partners should not mean disrupting patient care or delaying reimbursements. Smart RCM Billing plans each transition around your physical therapy practice’s existing workflows and payer requirements.

Parallel Claim Processing

Continue submitting claims through your existing clearinghouse while we validate billing rules, review payer responses, and prepare for a controlled transition.

Minimal Disruption to Daily Operations

Keep your front desk routines and therapists’ documentation processes intact while we manage billing tasks behind the scenes.

PT-Specific Transition Support

Maintain oversight of your revenue cycle while our team handles therapy coding, authorization issues, and denial follow-up.

Switch billing partners without putting your cash flow on hold.

Closing the Gap Between Notes and Payments

Smart RCM Billing works with compatible EHR and practice management systems, subject to a review of your existing setup, helping healthcare providers outsource billing while maintaining familiar workflows and minimizing operational changes.

Turn Treatment Records Into Stronger Claims

Physical therapy reimbursement depends on documentation that supports medical necessity, treatment time, billed units, and measurable progress. We help identify gaps between clinical notes and billing requirements before they lead to avoidable denials.

Documentation and Coding Alignment

Review CPT codes, timed units, modifiers, and treatment details against the services documented.

Plan-of-Care and Authorization Tracking

Monitor plan-of-care requirements, certification deadlines, and approved visit limits to help prevent billing interruptions.

Proactive Claim Review

Flag missing information and inconsistencies before submission, helping your team address documentation issues earlier.

Better alignment between therapy documentation and payer requirements means fewer preventable billing setbacks.

See Where Your Practice Stands

Smart RCM Billing gives physical therapy practices clearer insight into collections, outstanding claims, and recurring reimbursement problems, helping you make informed decisions about financial performance.

First-Pass Claim Quality

Monitor clean claim performance by payer, CPT code, and denial category to identify recurring errors and opportunities for improvement.

Accounts Receivable Visibility

Track A/R aging, prioritize overdue claims, and flag accounts approaching payer timely filing deadlines.

Collection and Underpayment Tracking

Compare payments against expected reimbursement, investigate underpayments, and identify opportunities to recover outstanding revenue.

Denial Trend Analysis

Categorize denials related to coding, eligibility, authorization, and documentation to address root causes and reduce repeat errors.

Make billing performance easier to understand, manage, and improve.

What Our Clients Say

I honestly didn’t realize how much of my day was being eaten up by small administrative tasks until SmartRCM’s VA took them over. Now I can finish my day without stress. These VA'S are also very good with patients, which was something I was nervous about initially.

Sarah Mitc. Family Medicine

With my previous billing company I faced a huge communication gap. But before switching our billing, I was constantly asking where payments were and why so many claims were sitting unresolved. SmartRCM actually communicated and kept me updated.

Michael Rey. Internal Medicine

My PT Practice was constantly facing billing issues. With SmartRCM, I can actually see which claims are pending, which denials need attention and where our A/R is getting stuck. That level of communication has made a big difference for us.

Jennifer Carter. Orthopedic & Sports Medicine

I had tried handling billing with an in-house person before, but honestly, it became one more thing I had to worry about. SmartRCM took over the billing follow-up and our virtual assistant also helps with scheduling and patient calls.

Ayesha Rah. Internal Medicine

We had months of old A/R. I wanted someone to actually work the accounts instead of just sending us another aging report. The SmartRCM team went through the backlog, contacted payers, worked the denials and kept me informed about what could realistically be recovered. Seeing payments come in from claims we thought were long gone was a very good feeling.

Frequently Asked Questions About Physical Therapy Billing

The 8-Minute Rule determines how many timed units a therapist can bill based on total skilled treatment minutes. 8–22 minutes equals one unit, 23–37 equals two, and so on in 15-minute increments after the first unit threshold. Documentation must support the exact minutes billed.
The KX modifier applies only when a patient’s cumulative therapy billing exceeds Medicare’s annual threshold and continued care remains medically necessary. The documentation must justify why the service is still skilled and necessary.
The GP modifier indicates that services were delivered by a physical therapist under a plan of care. The CQ modifier indicates that services were delivered by a physical therapist assistant under the therapist’s supervision, and reimbursement is reduced accordingly.
Many PT codes are time-based and require unit calculation rather than single-service billing. The 8-Minute Rule, KX threshold, and assistant modifiers are therapy-specific rules that do not apply to most other specialties.
What happens if a plan of care is not signed? Claims for services requiring a plan of care may be denied if the POC lacks a timely physician signature. Recertification deadlines must also be met for continued treatment to remain billable.
We also track denial patterns to address the root cause and prevent recurrence.
